Care Facilitator (HYBRID)
Full Time Position Available Immediately
Available Schedule: Monday-Friday 8am-4pm
The Starting Point Care Facilitator plays a crucial role in coordinating and managing the care of patients. They ensure effective communication, continuity of care, and optimal patient outcomes. They work closely with healthcare providers, patients, and their families to facilitate a patient-centered approach to healthcare delivery by fielding triage calls and coordinating care across services.
RN Tele-Triage (Fully Remote) - MUST have compact state RN licensure
Goal Start Date: 4/15/2024
Full Time: 2 (40 hours per week) positions available immediately
Available Schedules:
Tuesday-Saturday from 3pm –11pm EST (2pm – 10pm CST), or
Tuesday, Wednesday, Thursday from 1pm-11pm EST (12pm-10pm CST) and Saturday 9am-7pm EST (8am-6pm CST)
This Registered Nurse provides On-Call and Triage for the assigned population in accordance with triage protocols. This position remotely develops/implements care plans for persons served in the area of responsibility. The Tele-Triage RN assesses health status, identifies problems and needs of person(s) served in their assigned caseload, develops nursing care plans in collaboration with operations RN’s. You will complete chart reviews and monthly host home reviews and perform On-Bound calling to assigned populations/locations. This RN performs data analysis in multiple systems.
