The Psychiatric Rehabilitation Practitioner works as part of an Intensive Residential Treatment Services team that provides care, treatment and rehabilitation for persons served who are experiencing significant mental health/psychiatric symptoms; who are demonstrating significant functional impairments; and who may have co-occurring disorders.
The Targeted Case Manager provides Rule 79 Case Management services to eligible persons primarily referred directly from Hennepin County or self-referral, who have been diagnosed with a serious and persistent mental illness. TCM services assist with access to medical, social, and other services. Targeted Case Managers ensure continuity of care coordination and monitoring of services, discharge planning, housing support and intensive goal planning through the development of the Individual Community Support Plan.
