Physical Therapist - Full-Time - San Francisco, CA - Rehab Without Walls Neurorehabilitation
Who we are looking for:
- An experienced PT, seeking a primary therapist role from evaluation to discharge
- Caseload: 40 hours (Salary), 5-7 patients per week (typically, 5 patients with 6 hours of treatment each)
- Providing functional rehabilitation wherever life happens, whether at home, school, work, or in the community

The Hospice Clinical Pharmacist will support the operations of OnePoint Patient Care’s clinicalteam in our East Region and will report to the Vice President of Clinical Services.
Shift: 7 days on/ 7 days off: Monday-Thursday 8:00pm-6:00am, Friday 8:00pm-7:00 am, Saturday 6:00pm-7:00am, Sunday 6:00pm-6:00am
Plans and manages patient care according to each patient's needs. Interviews patients and records their medical history and physical condition. Obtains patient vital signs, including pulse, blood pressure, temperature, and respiration. Provides routine care for patients.
