The Patient Services Coordinator (Clinical) is responsible for scheduling home visits for field staff and coordinating patient care under the direction of the Clinical Manager. We’re looking for a Licensed Clinical Social Worker who believes real recovery happens when patients and families feel supported and empowered.
Under the general administrative direction of the reporting manager, the E-Referral Specialist will receive and respond to community, physician, patient and family request for home health. This position will record and process referral information as well as document in patient information database. The E-Referral Specialist will schedule registration and admission visits, prepare all required paperwork for admission as well as act as a liaison between patients, families, referral sources, physicians and admission staff. They will also assist in distribution of admission information and report to primary teams and physicians.
The Clinical Liaison will generate patient referrals and coordinate pre-screens, admissions, and serving as the primary contact to families and patients. The Clinical Liaison will also partner directly with the admissions team, program manager, and other departments to drive streamlined admission processes and to ensure positive relations within the community.
