Under the general administrative direction of the reporting manager, the E-Referral Specialist will receive and respond to community, physician, patient and family request for home health. This position will record and process referral information as well as document in patient information database. The E-Referral Specialist will schedule registration and admission visits, prepare all required paperwork for admission as well as act as a liaison between patients, families, referral sources, physicians and admission staff.
