Adoration Health

The Home Health Aide provides personal care and assist patients with activities of daily living in their residence under the supervision of a licensed clinician.

• Follow established policies and procedures.
• Implement the care plan as directed by the supervising clinician.
• Deliver personal care services including bathing, grooming, hygiene, and dressing assistance.
• Assist with vital signs monitoring, mobility support, feeding, toileting, and use of medical equipment; perform light housekeeping and ensure home safety.
• Address patient concerns professionally and respectfully.
• Communicate patient concerns to supervisor or designated staff.
• Maintain confidentiality of patient information.
• Report issues promptly and contribute to resolution efforts.
• Fulfill continuing education requirements and maintain competency.
• Apply infection control protocols and universal precautions consistently.
• Sanitize and transport patient care equipment; dispose of medical waste properly.
• Identify and report changes in patient condition.
• Document and report patient status accurately per agency and regulatory guidelines.
• Participate in scheduled meetings, conferences, and training sessions.
• Maintain and manage patient care equipment and supplies.

• High school diploma or equivalent
• Experience in home health or hospice care preferred
• Completion of Home Health Aide Training Program
• State certification/registration (if applicable)
• Current CPR certification
• Valid driver’s license and auto insurance
