Key Responsibilities
Perform comprehensive patient evaluations and develop individualized, goal-directed treatment plans.
Regularly reassess treatment plan effectiveness; actively participate in staffing meetings and family conferences.
Conduct discharge evaluations; recommend home and community safety modifications; provide referrals and educational resources to support continuity of care.
Communicate patient needs, progress, and outcomes clearly to the interdisciplinary team, physicians, patients, and families.
Collaborate across disciplines (PT, OT, SLP, nursing, neuropsychology) to coordinate care and optimize patient outcomes.
Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program.
Current, unrestricted Physical Therapist license in the state of practice.
Current Basic Life Support (BLS) or CPR certification, per state and facility requirements.
Willingness to complete required health screenings/exams per location or state policy.
1+ years of physical therapy experience preferred; new grads with strong neuro rehab clinical rotations will be considered.
Demonstrated knowledge of evidence-based rehabilitation techniques for complex neurological conditions (e.g., TBI, SCI, stroke, neurodegenerative disorders).
