Overview

The Specialty Pharmacist is responsible for coordinating and providing clinical pharmacy services to specialty and home infusion patients in collaboration with physicians, nurses, pharmacy staff, and other healthcare professionals. This role ensures safe, effective, and compliant medication therapy management while supporting exceptional patient care, operational excellence, and patient retention initiatives. The Specialty Pharmacist also serves as a clinical resource for patients, caregivers, providers, and internal teams while ensuring compliance with all applicable state, federal, accreditation, and company standards.

 

Shift: Monday-Friday 8:30am-5:30pm

Responsibilities

  • Clinical Responsibilities

    • Coordinate and facilitate new patient referrals and admissions in collaboration with nursing, customer service, reimbursement, intake, and distribution teams.
    • Evaluate physician medication orders for appropriateness of therapy including drug selection, dosing, route of administration, potential interactions, allergies, compatibility, margin considerations, and medical or pharmacy billing appropriateness.
    • Make clinical recommendations for therapy optimization or changes to prescribing physicians and facility staff when appropriate.
    • Conduct initial and ongoing clinical assessments of specialty patients and monitor patient outcomes to ensure safe and effective therapy management.
    • Evaluate and monitor drug therapy relative to the total needs of the patient in collaboration with physicians, nurses, and interdisciplinary healthcare teams.
    • Provide consultation and respond to inquiries from patients, caregivers, physicians, nurses, and other healthcare professionals regarding medications, specialty therapies, side effects, administration techniques, and treatment plans.
    • Process, verify, and review specialty medications prior to dispensing and shipment to ensure accuracy, safety, and compliance.
    • Maintain accurate, timely, and thorough documentation of all clinical activities, patient communications, interventions, and therapy management within the patient medical record and pharmacy systems.
    • Monitor patient adherence and persistence to therapy while proactively identifying and implementing interventions to improve patient retention and outcomes.
    • Participate in patient education activities related to specialty medications, administration, storage, side effect management, and therapy expectations.

    Operational & Compliance Responsibilities

    • Ensure compliance with all applicable federal and state pharmacy laws, regulations, and professional standards governing pharmacy practice.
    • Adhere to all Amerita policies, procedures, accreditation standards, and quality assurance initiatives including URAC, ACHC, and other applicable specialty pharmacy standards.
    • Participate in continuous quality improvement, performance improvement, and patient safety initiatives.
    • Support pharmacy operational workflow and collaborate across departments to ensure timely and efficient patient care services.
    • Oversee pharmacy technician functions in accordance with state Board of Pharmacy regulations and company policies.
    • Maintain all required licensure, certifications, and competency requirements necessary to perform the role.
    • Participate in on-call, weekend, holiday, or after-hours pharmacy responsibilities as required.

Qualifications

  • Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy required.
  • One (1) or more years of specialty pharmacy, infusion pharmacy, home infusion, or related clinical pharmacy experience preferred.
  • Experience with specialty medications, reimbursement processes, and patient management preferred.
  • Previous experience in a high-volume or fast-paced healthcare environment preferred.

LICENSE/CERTIFICATION/OTHER SPECIAL REQUIREMENTS

·         Required

  • Active and unrestricted Registered Pharmacist license in state(s) of practice.
  • Ability to obtain and maintain additional state licensure as required by business needs.

·         Preferred

  • Certified Specialty Pharmacist (CSP) credential through the National Association of Specialty Pharmacy (NASP).
  • Immunoglobulin Certified Pharmacist (IgCP) credential through the Immunoglobulin National Society (IgNS).

**To perform this role will require frequently sitting, standing, walking and typing on a keyboard with fingers, and occasionally bending, reaching, and climbing (stairs/ladders). The physical requirements will be the ability to push/pull and lift/carry 1-10 lbs**
