The Quality Nurse Care Mgr supports population health and value-based care initiatives by identifying, tracking, and validating closure of quality gaps across assigned patient populations. This role works closely with Care Management and Value-Based Operations teams to ensure preventive and chronic care measures are addressed and accurately documented.

• Identify and track open quality gaps using registries, chase lists, and population health tools
• Prioritize gaps based on program and contract requirements
• Validate closure of quality measures through chart review
• Support coordination of outreach and follow-up activities
• Review clinical records to confirm completion of screenings, labs, and preventive services
• Verify documentation accuracy and completeness
• Maintain quality trackers and dashboards
• Collaborate with Care Management and Value-Based Operations teams
• Participate in quality huddles and performance reviews
• Support audit preparation and quality reporting activities
• Provide feedback on documentation requirements
• Assist with onboarding and training related to quality workflows
• Attend care huddles and review all open gaps

• Minimum 2–4 years of relevant healthcare experience (population health, care management, quality improvement, or value-based care preferred)
• Experience with chart review and documentation validation
• Experience with ACOs, MA plans, or risk-based models
• Familiarity with EMRs, registries, and quality reporting tools
• Active, unrestricted compact LPN or RN license
• Strong organizational and attention-to-detail skills
• Ability to follow standardized protocols
• Effective communication skills
• Collaborative and team-oriented
• Comfortable working with metrics and dashboards
• Travel up to 25%
