Rehab in Motion
The Outpatient Site Manager (Rehab in Motion) is directly responsible for managing the business and clinical operations of assigned treatment facility. The Site Manager assures care and services provided by the team are in compliance with state and federal regulatory requirements of accrediting bodies and provided to each patient in a safe, cost-effective manner that meets applicable standards of practice.
Promotes, assures, and implements a philosophy of patient-centered treatment services while delivering and providing leadership, accountability and guidance as needed.
Facilitates and oversees the care coordination and interdisciplinary communication activities of team members including patient conferences (“staffings”) and family conferences.
Ensures timely conversion of referrals to admissions and facilitation of an effective patient
admission process to achieve or exceed all admission goals.
Oversees patient scheduling as required for all services, optimizing efficiencies in resource utilization.
Assures all patients are treated in accordance with physician’s order for care.
Actively communicates with all stakeholders involved in patient treatment; referral sources, patients and families.
Based on business needs, leads local recruitment, hiring, and development clinical and administrative staff to ensure the delivery of safe and consistent quality care to all patients.
Gather data for assigned area(s) and prepare written reports. Use the collected data to identify and areas of concern and implement quality improvement plans to address the concerns.
Serves as the primary local point contact person for case management and contract monitoring personnel assigned to the local area Agency on Aging, DSHS Home and Community Services, health care providers, and the Dept. of Health. Attend and participate in area meetings, case staffing, and address local contract concerns as needed.
Conducts chart audits to ensure that all federal and state law requirements and conditions of participation for Medicare and Medicaid are met.
Communicates effectively with other managers, administrative, quality, HR, business development, senior leadership, and clinical teams.
Bachelors’ degree preferred or at least 3 years’ clinical or rehab supervisory experience. Requires proven interpersonal, supervisory, and management skills with the ability to communicate effectively.
Occupational Therapy Assistant (OTA) or Physical Therapist Assistant (PTA)
Knowledge/Training: Knowledge of various funding sources and regulations as applied to brain injury rehab services. Demonstrates knowledge and competence in advanced practice clinical skills, including goals driven therapy and utilization management.
Computer Skills: Must be familiar with general use and functions of the computer, such as, usernames and password concepts; internet; e-mail; navigation of computer desktop or laptop, including starting programs, using files, and windows, effectively use navigation buttons and tool bars; ability to self-manage online HR services and online training programs.
Transportation: Must have valid automobile insurance and reliable transportation.
