SpringPath Mental Health Services
The Peer Support Specialist works within an interdisciplinary team intended to provide intensive, frequent, wrap-around services to individuals who have a diagnosis of serious and persistent mental illness. The Peer Support is assigned to persons served individualized treatment teams and work along-side other specialists in carrying out peer support interventions. As part of the person(s) served individualized treatment team, the Peer Support is responsible for conducting assessments that monitor the person(s) served’ s goals and functioning on an ongoing basis. The Peer Support typically sees approximately 16 to 20 persons served within 1 working week and is expected to communicate interactions with the person served at daily team meetings and documenting these interventions in the Electronic Health Record.
