Overview
Private Duty LPN – G-Tube Care (Hornbeak, TN)
Overview:
Join our caring team as a Private Duty LPN, providing in-home care split between Hornbeak, TN, and Jackson, TN. You’ll work the first half of the month in Jackson and the second half in Hornbeak, enjoying variety while making a meaningful difference.
Location & Schedule:
- Hornbeak, TN: 15th through the 30th of each month
- Week 1: Monday, Tuesday, Thursday, Friday (7:30am–5:30pm)
- Week 2: Monday, Tuesday, Saturday, Sunday (7:30am–5:30pm)
- Jackson, TN: 1st through the 15th of each month
- Same schedule split applies

Qualifications:
- Current LPN license
- G-Tube care experience required
- CPR certification
Responsibilities
- Perform selected nursing procedures within the scope of LPN practice
- Provide patient and family teaching according to plan outlined by the RN
- Monitor patient's vital signs and make specific measurements and observations
- Monitor and record intake and output. Report significant changes immediately
- Observe, assess and record patient's physical and mental condition and reports any changes
- Administer medications as instructed and as ordered by the physician and instructed by the RN and document the patient's file accordingly
- Attend and participate in multi-disciplinary patient case conferences, staff meetings and in-service education programs as assigned
Qualifications
- Licensed Practical Nurse / LPN license in good standing
- Previous private duty, home health, hospice, hospital or skilled nursing experience preferred
- Active CPR certification
- Familiarity with electronic health records
- Strong problem solving and critical thinking skills
- Reliable transportation, valid driver's license and current automobile insurance
