You will make a difference in our patient’s lives by providing safe patient care and delivering individualized physician care plans to ensure our patients have an optimal care experience while working as part of an interdisciplinary team, supporting patients and their families. Coverage area: Garfield County

Schedule: PRN

• Performing personal care needs
• Planning and preparing nutritious meals and assisting in feeding the patient when necessary
• Taking and recording temperatures, pulse, respiration and blood pressure as ordered
• Assisting in ambulation and exercise, as instructed
• Assisting patient in self-administration of medication
• Meeting safety needs of patient and using equipment safely and properly
• Assessing and reporting on patient’s condition and significant changes to the assigned nurse
• Keeping the patient’s living area clean and orderly
• Ensuring compliance with all state and federal legal and regulatory requirements
• Additional responsibilities as assigned

• At least 18 years of age
• Completion of Certified Nurse Aide/State Tested Nurse Aide training and current licensure to practice in state of operation
• 1 year CNA/STNA experience required
• Experience in home health or a similar setting preferred
• Self-directing with ability to work to work with little direct supervision
• Commitment to clinical and documentation excellence
