Private Duty LPN - Night Shift (39 hrs/week)

Overview:
Provide one-on-one, in-home care for patients in a supportive and flexible work environment. This role involves trach, vent, and G-Tube care, offering a meaningful opportunity to deliver personalized care in a comfortable home setting.

 

Location:
Rutherford County, TN or Dyer County, TN

Highlights:

• Training Available: Comprehensive training is offered for trach/vent certification.
• Personalized Care: Focus on building meaningful patient relationships in a one-on-one setting.

• Perform selected nursing procedures within the scope of LPN practice
• Provide patient and family teaching according to plan outlined by the RN
• Monitor patient's vital signs and make specific measurements and observations
• Monitor and record intake and output. Report significant changes immediately
• Observe, assess and record patient's physical and mental condition and reports any changes
• Administer medications as instructed and as ordered by the physician and instructed by the RN and document the patient's file accordingly
• Attend and participate in multi-disciplinary patient case conferences, staff meetings and in-service education programs as assigned

• Licensed Practical Nurse / LPN license in good standing
• Previous private duty, home health, hospice, hospital or skilled nursing experience preferred
• Active CPR certification
• Familiarity with electronic health records
• Strong problem solving and critical thinking skills
• Reliable transportation, valid driver's license and current automobile insurance
