Private Duty LPN – Full-Time Day Shifts
Location: Kenton, TN
Overview:
Adoration Home Health & Hospice is seeking dedicated and compassionate Licensed Practical Nurses (LPNs) to provide specialized one-on-one care for patients with G-Tube needs in their homes. This full-time role offers a unique rotating schedule, providing flexibility and meaningful patient interaction.
Schedule:
- Day Shifts: 6:00 AM – 6:00 PM
- Rotating ¾ Split:
- Week 1: 36 hours
- Week 2: 48 hours
- Every other weekend
Positions Available:
- Two LPNs needed for day shifts

Qualifications:
- Active LPN license in Tennessee
- Current CPR certification
- Experience with G-Tube care preferred
- Strong organizational and communication skills

Responsibilities
- Perform selected nursing procedures within the scope of LPN practice
- Provide patient and family teaching according to plan outlined by the RN
- Monitor patient's vital signs and make specific measurements and observations
- Monitor and record intake and output. Report significant changes immediately
- Observe, assess and record patient's physical and mental condition and reports any changes
- Administer medications as instructed and as ordered by the physician and instructed by the RN and document the patient's file accordingly
- Attend and participate in multi-disciplinary patient case conferences, staff meetings and in-service education programs as assigned
Qualifications
- Licensed Practical Nurse / LPN license in good standing
- Previous private duty, home health, hospice, hospital or skilled nursing experience preferred
- Active CPR certification
- Familiarity with electronic health records
- Strong problem solving and critical thinking skills
- Reliable transportation, valid driver's license and current automobile insurance
