Private Duty LPN – Geriatric Female Patient – Chattanooga, TN

Adoration Health is looking for an experienced Private Duty LPN to provide compassionate, one-on-one care for a geriatric female patient in Chattanooga (37404). If you have expertise in vent/trach care and a passion for making a meaningful impact, this role is for you.

Key Responsibilities:

Deliver personalized, in-home patient care with a focus on comfort and well-being.
Skillfully manage vent/trach systems and catheter care.
Collaborate closely with the healthcare team to uphold the highest standards of care.

Qualifications:

3+ years of vent/trach experience (required)
Active Tennessee LPN license
Prefer experience in geriatric care

Responsibilities

• Perform selected nursing procedures within the scope of LPN practice
• Provide patient and family education according to plan as outlined by the RN
• Monitor patient’s vital signs and make specific measurements and observations
• Monitor and record intake and output, and report significant changes immediately
• Observe, assess and record patient’s physical and mental condition and report any changes
• Administer medications as instructed and ordered by the physician and instructed by the RN
• Attend and participate in staff meetings and in-service education programs, as assigned

Qualifications

• Active LPN license in good standing
• Previous home health, private duty, skilled nursing or hospital experience preferred
• Ability to build rapport with patients and family members
• Strong problem solving and critical thinking skills
• Reliable transportation or valid driver’s license with current automobile insurance
