Adoration Health is seeking a skilled Licensed Practical Nurse (LPN) for a full-time position in Hornbeak, TN. This role involves providing nighttime care for a patient requiring trach and G-tube management. The schedule fluctuates between Sunday to Friday or Monday to Friday, depending on the family's needs. Schedule:
- 50 hours per week
- Fluctuating between Sunday-Friday or Monday-Friday
- Night shifts: 8:00 PM - 6:00 AM
Responsibilities:
- Provide high-quality nursing care, including trach and G-tube management, in a home care setting.
- Monitor and document patient conditions, ensuring timely communication with the healthcare team.
- Administer medications and perform routine nursing procedures.
- Collaborate with the patient’s family and healthcare providers to ensure comprehensive care.

Responsibilities
- Perform selected nursing procedures within the scope of LPN practice
- Provide patient and family teaching according to plan outlined by the RN
- Monitor patient's vital signs and make specific measurements and observations
- Monitor and record intake and output. Report significant changes immediately
- Observe, assess and record patient's physical and mental condition and reports any changes
- Administer medications as instructed and as ordered by the physician and instructed by the RN and document the patient's file accordingly
- Attend and participate in multi-disciplinary patient case conferences, staff meetings and in-service education programs as assigned
Qualifications
- Licensed Practical Nurse / LPN license in good standing
- Previous private duty, home health, hospice, hospital or skilled nursing experience preferred
- Active CPR certification
- Familiarity with electronic health records
- Strong problem solving and critical thinking skills
- Reliable transportation, valid driver's license and current automobile insurance
