Private Duty LPN - Night Shifts (Vent/Trach) | East Ridge, TN

 

Schedule: Tuesday - Thursday, Rotating + One Monday per Month
Shift Hours: 6:00 PM - 6:00 AM

We’re seeking an experienced nurse to provide one-on-one, in-home care for a ventilator and trach-dependent patient in East Ridge, TN. What You’ll Do:

• Provide specialized care for a patient with complex needs, including vent/trach and G-tube management.
• Monitor and assess patient condition throughout the night to ensure safety and comfort.
• Maintain thorough documentation and collaborate with the care team to uphold a high standard of care.

Qualifications:

• Active LPN license in Tennessee.
• Minimum of 1 year of experience with vent/trach-dependent patients.
• Strong clinical skills and ability to manage high-acuity care needs.

• Perform selected nursing procedures within the scope of LPN practice
• Provide patient and family education according to plan as outlined by the RN
• Monitor patient’s vital signs and make specific measurements and observations
• Monitor and record intake and output, and report significant changes immediately
• Observe, assess and record patient’s physical and mental condition and report any changes
• Administer medications as instructed and ordered by the physician and instructed by the RN
• Attend and participate in staff meetings and in-service education programs, as assigned

• Active LPN license in good standing
• Previous home health, private duty, skilled nursing or hospital experience preferred
• Ability to build rapport with patients and family members
• Strong problem solving and critical thinking skills
• Reliable transportation or valid driver’s license with current automobile insurance
