Private Duty Licensed Practical Nurse (LPN) – Geriatric Female – Chattanooga, TN (37404)

Adoration Health is looking for an experienced Private Duty LPN to provide care for a geriatric female patient in Chattanooga, TN (37404). This role requires at least 3 years of expertise in vent/trach care.

Primary Duties:

  • Deliver personalized, one-on-one care in the patient’s home.
  • Skillfully manage vent/trach systems and handle catheter care.
  • Work closely with the healthcare team to ensure top-quality care is provided.

Requirements:

  • A minimum of 3 years of direct experience with vent/trach management (required).
  • Active LPN license in Tennessee.
  • Strong experience in geriatric patient care.

Responsibilities

• Perform selected nursing procedures within the scope of LPN practice
• Provide patient and family education according to plan as outlined by the RN
• Monitor patient’s vital signs and make specific measurements and observations
• Monitor and record intake and output, and report significant changes immediately
• Observe, assess and record patient’s physical and mental condition and report any changes
• Administer medications as instructed and ordered by the physician and instructed by the RN
• Attend and participate in staff meetings and in-service education programs, as assigned

Qualifications

• Active LPN license in good standing
• Previous home health, private duty, skilled nursing or hospital experience preferred
• Ability to build rapport with patients and family members
• Strong problem solving and critical thinking skills
• Reliable transportation or valid driver’s license with current automobile insurance
