Position Overview: As an LPN Float, you will deliver high-quality nursing care across multiple in-home settings. Your primary responsibility will be to ensure the well-being of our patients while offering flexible and comprehensive care. Key Responsibilities:
- Provide exceptional nursing care to patients in their homes, adhering to individualized care plans.
- Administer medications, perform wound care, and assist with other clinical interventions as needed.
- Collaborate with the healthcare team to update and maintain accurate patient records.
- Educate patients and their families on managing their health and wellness.
- Adapt to various home care environments and patient needs, demonstrating flexibility and responsiveness.
Qualifications:
- Current Licensed Practical Nurse (LPN) license in the state of Tennessee.
- Minimum of 1 year of nursing experience, preferably in home health or acute care settings.
- Strong clinical skills and the ability to work independently.
- Excellent communication and interpersonal skills.
- Valid driver's license and reliable transportation.

Responsibilities
• Perform selected nursing procedures within the scope of LPN practice
• Provide patient and family education according to plan as outlined by the RN
• Monitor patient’s vital signs and make specific measurements and observations
• Monitor and record intake and output, and report significant changes immediately
• Observe, assess and record patient’s physical and mental condition and report any changes
• Administer medications as instructed and ordered by the physician and instructed by the RN
• Attend and participate in staff meetings and in-service education programs, as assigned
Qualifications
• Active LPN license in good standing
• Previous home health, private duty, skilled nursing or hospital experience preferred
• Ability to build rapport with patients and family members
• Strong problem solving and critical thinking skills
• Reliable transportation or valid driver’s license with current automobile insurance
