What You’ll Do:

• Deliver direct, compassionate nursing care to hospice patients in their homes or facility settings

• Collaborate with physicians, social workers, spiritual counselors, and other team members to create and follow individualized care plans

• Educate patients and families on care processes and provide emotional support throughout the journey

What You’ll Need:

• Active LVN license in good standing

• Valid driver's license, active auto insurance, and a reliable personal vehicle for work-related travel

• Provide direct patient care to hospice patients, including appropriate pain/symptom management
• Implement nursing practice following a comprehensive assessment and plan of care
• Provide accurate and timely documentation of patient services
• Assess and provide patient and family/caregiver education
• Participate in coordination of hospice services
• Provide appropriate pain/symptom management and evaluate patient response to treatments/medications
• Participate in professional growth and development opportunities and quality assessment activities
• Ensure compliance with all state and federal legal and regulatory requirements
• Additional responsibilities as assigned

• Graduate of an accredited practical or vocational nursing program with current licensure to practice in state of operation

• Three years nursing experience. Community health/hospice or medical/surgical experience is

  preferred

• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence

• Must be a licensed driver with an automobile that is insured in accordance with state and/or

  organization requirements and is in good working order.

• Possess and maintain current CPR certification.
