Overview

The Clinical Liaison PRN will generate patient referrals and coordinate pre-screens, admissions, and serving as the primary contact to families and patients.  The Clinical Liaison will also partner directly with the admissions team, program manager, and other departments to drive streamlined admission processes and to ensure positive relations within the community. 

Responsibilities

  • Functions within and promote general policies, stated beliefs, and philosophy of the organization
  • Promotes the Rehab Without Walls philosophy and vision to ensure patient satisfaction
  • Provides referral sources with progress reports and other appropriate information as required and permitted within HIPAA guidelines
  • Supports development of pre-screen and related admissions and evaluation resources and processes for potential admissions
  • Identifies special needs of potential patients, schedule, and attend pre-admission and follow-up planning meetings
  • Coordinates admission process with the medical staff or referring entity and provide feedback to Admissions Coordinator and appropriate treatment team
  • Educates the community regarding brain injury and advocate for services needed during the pre-screening and admissions process
  • Creates and maintains information for evaluation and scheduling of pre-admission evaluation visit
  • Serves as back up or lead to gather pre-admission information for evaluation and scheduling of pre-admission evaluation visit
  • Contacts patient/family and schedule pre-admission evaluation visit within 24 hours of referral
  • Travels as needed
  • Other duties including special projects as assigned

Qualifications

  • Degree in Nursing or related therapy field (SLP, OT, PT, LCSW)
  • Maintain or able to obtain an unrestricted license to practice in their home state
  • A minimum of five years’ experience in health care accreditation with prior work experience in CARF accredited operation preferred
  • A minimum of five years’ experience in quality, data management, clinical informatics, or electronic medical records [EMR]
  • Demonstrates effective and professional interpersonal, verbal, and written communication skills
