Responsible for coordinating all aspects of the patient intake process, establishing and maintaining positive relationships with customers and referral sources as well as staff. Responds to customer requests and concerns, works closely with Authorization Specialist to manage the insurance verification processes and assist with the authorization processes. Communicates scheduling changes between office and field staff accurately and timely.
• Receive and process new patients for admission to our agency using EMR system.
• Coordinates care by assigning case manager and clinician(s) to ensure 48-hour admission.
• Communicates with various healthcare professionals to provide highest level of care.
• Use HIPAA compliant processes to receive and process orders/faxes for new and current patients.
• Performs follow up calls to patients and caregivers to offer clear and supportive communication.
• Performs other various coordination duties to support day to day work flow.
• Maintains positive attitude when dealing with others.
• Organized and able to multi task, with meticulous documentation.
• Goal oriented, proactive and takes initiative and demonstrates flexibility.
• Participates in team meetings, events and office culture.
• Promotes agency policies and procedures.
• Communicates with Supervisor and notifies promptly of adverse findings or unfavorable situations.
• Demonstrates proper knowledge of State and federal and payer rules/regulations.
• May occasionally be assigned to provide patient care.
• Participates in the On Call after hours rotation if needed.
• Presents neat, professional appearance.
• Performs other duties as assigned.
• Graduate of an approved Nursing education program.
• Current, active license to practice as LPN, with at least 2 years of successful experience in health care (preferably home care), practicing as an LPN.
• Demonstrates excellent oral and written communication skills and problem-solving capabilities.
• Knowledge of home health regulations, eligibility and private payer practices.
• Demonstrates ability to appropriately prioritize, organize, display assertiveness, flexibility and cooperation in performing job responsibilities.
• Demonstrates ability to get along well with others.
• Must possess and maintain current, valid driver’s license, reliable automobile transportation, proof of current automobile insurance, current CPR certification by either American Heart Association or Red Cross.
• Basic computer skills and office technology required.
• Possesses great attention to detail, accuracy, and ability to complete assignments in an efficient and timely manner.
